HOW TO OBTAIN A SICK NOTE FROM YOUR GP
If you are absent from work due to illness for seven days or less then you do not require a sick note from your GP. You can self–certify your illness using an SC2 form from HMRC. This form can normally be accessed from your employer or you may download it below.

The GP will provide you with a Statement of Fitness for Work (‘Fit Note’) if you are still not well enough to work. If you’re absent from work due to illness for more than seven days, your employer will most likely request this statement as evidence to support payment of Statutory Sick Pay (SSP).
To obtain a sick note, please call 0141 339 5513 and wait to speak to the receptionist. You will be asked to state the start and end date for your period of sickness. Unless stated otherwise, you will be contacted by the Practice staff via telephone to collect your sick note when ready.
